This form should be used by vendors/suppliers that didn’t get registered under an exhibitor booth or if you simply want to attend. This should also be used if you are providing a workshop and you aren’t registered through an exhibit.
(those that do not register by 10/2/16 and plan to register on-site will be charged $75/representative)
